St. Eugene Sports Information

A. Medical Information and Emergency Consent (Form 6145.2a) will need to be filled out for each sport. This
form will be kept with the coach of record for the particular sport just in case there is an emergency during
a game and or practice.
B. Risk Acknowledgement and Consent to Participate (Form 6145.2b) is also required for each sport. (This form
replaces the St. Eugene Specific indemnity form).
C. Medical Physical Forms (Form 6145.2c). Physical forms are good for two years from examination date. D. New Sportsmanship Pledge Forms (as of August 2006) consists of three separate documents that need to be signed by the coach, student athlete, and parent. See specific forms for details. click here All of these forms are required by the Archdiocese in order to participate in Archdiocese sanctioned leagues and should be turned into the coach of record for your particular sport. They should NOT be turned into the school, parish or CYM office! |
