The following forms are available from the school office or may be downloaded here.
We must have the parent consent form, and the physician authorization form if it is a prescription medication, completed before any medication is administered to your child at St. Eugene School. The medication, both prescription and over-the-counter, must be in its original container, clearly displaying the child's first and last name, name of medication, instructions on dosage and when it is to be given. |
